As of May 12, 2026, PCOS has officially been renamed. The new name is PMOS: Polyendocrine Metabolic Ovarian Syndrome. The change was published in The Lancet by an international consensus group = led by Professor Helena Teede of Monash University. It took 14 years of wor= k and involved more than 14,000 patients and clinicians from every region o= f the world to get this across the finish line. The condition itself didn=E2=80=99t change. The biology is the same. What c= hanged is the official recognition that this is not a disease of the ovarie= s. It=E2=80=99s a whole-body metabolic and hormonal condition that happens = to show up in the ovaries. That distinction is the entire point. Why The Old Name Was A Problem The old name pointed at the wrong thing. =E2=80=9CPolycystic ovary syndrome=E2=80=9D made it sound like the problem = was cysts on the ovaries. Two issues with that. First =E2=80=94 those aren=E2=80=99t actually cysts. When an ultrasound sho= ws the =E2=80=9Cpolycystic=E2=80=9D appearance, what you=E2=80=99re seeing = are eggs that started developing and got stuck partway through. They never = matured. They never released. They just stalled. They look like little flui= d-filled sacs, which is why they got called cysts, but they=E2=80=99re not = the same thing as the cysts you=E2=80=99d think of in other conditions. Second =E2=80=94 even those stalled eggs aren=E2=80=99t the actual disease.= They=E2=80=99re a symptom. The real problem is happening somewhere else en= tirely, and the ovaries are just where you can see it on a screen. Think of it like this: if your car keeps overheating, the temperature gauge= isn=E2=80=99t the problem. It=E2=80=99s telling you something is wrong wit= h the engine. Calling it =E2=80=9Cpolycystic ovary syndrome=E2=80=9D is lik= e calling your car trouble =E2=80=9Chigh temperature gauge disease.=E2=80= =9D It points at the symptom, not the cause. This matters because the wrong name has led to the wrong care for decades. = About 1 in 8 women globally have this condition. That=E2=80=99s around 170 = million people. And up to 70 percent of them are walking around undiagnosed= =E2=80=94 partly because the name made everyone think it was a niche ovary= issue instead of a major whole-body metabolic problem. What=E2=80=99s Actually Happening This is the part most people never get explained clearly. Here=E2=80=99s th= e simple version. There are four things going wrong, and they all push on each other in a loo= p. Problem 1: Insulin stops working properly Insulin is the hormone that tells your cells to absorb sugar from your bloo= d. In PMOS, the cells stop listening. So your body=E2=80=99s response is to= make more insulin to force the message through. Now you=E2=80=99ve got way= too much insulin floating around in your blood all the time. This is called insulin resistance, and it=E2=80=99s the root of the whole c= ondition. Problem 2: Too much insulin tells the ovaries to make too much testosterone Here=E2=80=99s something most people don=E2=80=99t know =E2=80=94 insulin d= oesn=E2=80=99t just regulate blood sugar. It also talks directly to the ova= ries. When insulin levels are high, it signals the ovaries to crank out ext= ra testosterone and other male-pattern hormones (called androgens). That=E2=80=99s why PMOS shows up as acne, unwanted hair growth, and hair th= inning. It=E2=80=99s not a skin problem or a hair problem. It=E2=80=99s an = insulin problem showing up on the surface. Problem 3: The brain=E2=80=99s cycle signals get scrambled Your brain controls your menstrual cycle by sending out timed hormone pulse= s. In PMOS, those pulses get out of rhythm. The brain sends out too much of= one signal (LH) and not enough of another (FSH). FSH is the signal that tells eggs to mature. Without enough of it, eggs sta= rt developing but never finish. Problem 4: Eggs stall instead of ovulating Because the FSH signal is weak, eggs get stuck in development. They don=E2= =80=99t mature, they don=E2=80=99t release, and ovulation doesn=E2=80=99t h= appen the way it should. That=E2=80=99s what creates the =E2=80=9Cpolycysti= c=E2=80=9D look on ultrasound =E2=80=94 a bunch of half-developed eggs sitt= ing there waiting. No ovulation also means no progesterone, which means cycles become irregula= r or skip entirely. The trap: all four problems make each other worse Insulin resistance drives high testosterone. High testosterone makes insuli= n resistance worse. The scrambled brain signals push both of them further. = The ovaries get caught in the middle. The ovaries aren=E2=80=99t broken. They=E2=80=99re responding exactly the w= ay you=E2=80=99d expect them to in this hormonal environment. Fix the envir= onment and the ovaries usually start working again. This is why =E2=80=9Cpolyendocrine metabolic=E2=80=9D is the right name: Poly means many Endocrine means hormone systems Metabolic means how your body processes energy There are many hormone systems involved, and the engine running underneath = all of it is metabolic. Why The Name Change Matters A name change sounds cosmetic. It=E2=80=99s not. Here=E2=80=99s what actual= ly shifts when the framing shifts. Doctors think differently When a doctor hears =E2=80=9Cpolycystic ovary syndrome,=E2=80=9D they think= : do an ultrasound, look at the ovaries, refer to a gynecologist. The worku= p focuses on reproductive structure. When a doctor hears =E2=80=9Cpolyendocrine metabolic ovarian syndrome,=E2= =80=9D they think: check insulin, check cholesterol, check cardiovascular r= isk, check the full hormonal picture. The ovary findings become one piece o= f a much bigger investigation. That=E2=80=99s a completely different standard of care. And it=E2=80=99s th= e one that actually matches the biology. Research gets better funded Research grants and journals categorize conditions by name. =E2=80=9CReprod= uctive disorders=E2=80=9D is a smaller, historically underfunded research p= ool. =E2=80=9CEndocrine and metabolic disorders=E2=80=9D is a much bigger o= ne =E2=80=94 the same pool that funds diabetes, obesity, and heart disease = research. Just by changing the name, this condition gets access to far more research = money, attention, and crossover with other metabolic science. Stigma drops The old name forced reproductive language into every conversation about it.= In many cultures, that created shame and made people less likely to seek c= are or talk about it openly. The new name is medical and metabolic. It describes a body system, not a re= productive label. Patients ask better questions When you know your condition is metabolic, you start tracking the right thi= ngs. You ask about insulin, A1C, fasting glucose, lipid panels =E2=80=94 no= t just whether your cycle is regular. You see it as a long-term health cond= ition with cardiovascular and metabolic implications, not just a fertility = issue. What Didn=E2=80=99t Change The diagnosis itself stayed exactly the same. You still need two out of thr= ee of these: Irregular or absent ovulation High androgens (testosterone-type hormones) Polycystic ovaries on ultrasound, or elevated AMH (a hormone that reflects = egg supply) For teenagers, the third criterion isn=E2=80=99t used by itself because pol= ycystic-looking ovaries can be normal in adolescence. What also didn=E2=80=99t change immediately is treatment. But the framing n= ow points clinicians toward addressing the metabolic root =E2=80=94 insulin= sensitivity, body composition, long-term cardiovascular risk =E2=80=94 rat= her than just managing symptoms. The Rollout This is a managed 3-year transition. Both names will be in use during that = time. That=E2=80=99s normal and expected. Medical records, textbooks, and training programs are migrating now. The of= ficial International Guideline (used in 195 countries) updates in 2028. The= WHO is adding PMOS to the official disease classification system. You=E2=80=99ll see =E2=80=9CPCOS/PMOS=E2=80=9D or both names together for a= while. Eventually it=E2=80=99ll just be PMOS. Why This Matters For Us For anyone paying attention to metabolic research, this rename is huge. For years, the research community has been saying this is a metabolic condi= tion. Now the official name says it too. That means the broader conversation around insulin sensitivity, GLP-1 mecha= nisms, mitochondrial function, and metabolic optimization is now formally r= elevant to PMOS. Expect a lot more crossover between the metabolic research= space and the PMOS literature over the next few years.
